Dental and vision are a key part of your overall health.
Dental
You have a choice of two dental plan options (Standard and Premium Plan) through BlueCross BlueShield of Illinois (BCBSIL). Key features include:
- Preventive care covered at 100% – you pay $0
- Basic restorative covered at 80% after you meet the deductible
- Major restorative covered at 50% after you meet the deductible
- Orthodontia covered at 50%, lifetime maximum varies based on the plan option you choose
- How much the plan will pay toward the cost of your care each year (annual benefit maximum) varies by plan
Enhanced Features of Our Dental Plans
- Teledentistry – You have access to virtual visits, powered by Teledentistry.com, if you or a covered dependent need access to a dentist after hours or while traveling, or if you’re having a dental emergency and can’t see your dentist.
- Additional benefits for diabetes, heart disease and pregnancy – Outside of the annual routine services, you may be eligible for one additional routine cleaning, periodontal scaling and root planing or periodontal maintenance for you and your covered dependents.
Vision
You have a choice of two vision plan options (Standard and Premium Plan) through EyeMed. Coverage includes:
- Eye exams
- Glasses (lenses and frames) or contact lenses
- Discounted pricing on certain vision services
Make The Most of Your Vision Plan
- Eye360 – Your EyeMed vision plan provides you with additional benefits and savings at PLUS Providers, which has in-network eye doctors that offer additional perks, like $0 eye exams and higher frame allowances.
- Additional discounts – Both vision plans offer you the opportunity to save through discounts on glasses, vision correction surgery and hearing aids.
- Additional benefits for diabetes – If you or a covered dependent has Type 1 or Type 2 diabetes, you have access to $0 copays for diabetic care services, including medical follow-up eye exams, extended ophthalmoscopy, fundus photography exam, gonioscopy and scanning laser services.
Vision Abroad
If you forget your glasses or they break while traveling, our plan’s vision abroad services provide you or a covered dependent with temporary, adjustable eyewear the next day.
